Mastering the OET: A Comprehensive Guide to the Exam Format

The OET is a specialized English proficiency exam for healthcare professionals. Unlike general English tests, it focuses on medical contexts and real-world clinical scenarios.

What is the OET?

The Occupational English Test (OET) is an internationally recognised English language test designed specifically for healthcare professionals. Unlike general English exams such as IELTS or PTE, OET uses medical contexts — patient consultations, clinical notes, referral letters — to assess your communication skills.

OET is accepted by regulatory bodies in the UK, Australia, New Zealand, Ireland, Dubai, Singapore, and Canada. For Nepali nurses and doctors aiming to register abroad, OET is often the preferred — and sometimes the only — route to registration.

The Four Sub-Tests

OET measures your English ability across four key areas: Listening, Reading, Writing, and Speaking. Each sub-test is set in a healthcare context, making the content directly relevant to your professional life.

Listening and Reading are shared across all health professions. Writing and Speaking are profession-specific — nurses receive nurse-patient scenarios, doctors receive doctor-patient scenarios, and so on. This specialisation is what makes OET uniquely valuable for healthcare workers.

Scoring and Grading

OET results are reported as grades from A (highest) to E (lowest), with corresponding scores between 0 and 500. Most regulatory bodies — including the NMC (UK) and AHPRA (Australia) — require a minimum of Grade B (350+) in each sub-test.

It is important to note that you must achieve Grade B in ALL four sub-tests simultaneously. A Grade A in three sub-tests does not compensate for a Grade C in one. Planning your preparation around this requirement is essential.

How OET Differs from IELTS

IELTS Academic tests general academic English — you may encounter topics ranging from archaeology to economics. OET, by contrast, only presents healthcare scenarios. This means your preparation is far more targeted and, for working healthcare professionals, far more intuitive.

Many nurses and doctors who struggled with IELTS find OET significantly more manageable once they understand the format, because the vocabulary and situations mirror their daily professional experience.
